Florida Zika Testing Declines As Local Cases Rise

Yesterday, Florida reported 3 more local Zika cases (including two more outside of Wynwood), bringing the local total to 33.  Although Florida started two more investigations (on the two new confirmations outside of Wynwood), the reported totals for samples collected and tested indicates testing has dramatically slowed.

Two exports out of Florida have already been reported (in El Paso, Texas and New Taipai City, Taiwan) by travelers visiting Miami-Dade and Broward counties, and the number of exports is undoubtedly much higher than reported.

The limited Florida testing suggests that active transmission within Florida is not limited to Wynwood, which raises serious concerns about the restricted CDC travel warning (limited to Wynwood area)..

A new area of ongoing Zika transmission has been identified in Miami Beach, according to sources familiar with discussions that Florida health officials held with local public officials Thursday to alert them.

The Aug 23 update of active Zika investigation raises serious concerns about the cases in Miami Dade outside of Wynwood.  For three of the investigations #1, #3, #4 using Florida DoH numbering, each has 2 or more pending cases that were collected on August 17 or earlier (2,5, 2 pending samples, respectively).

Moreover, for the other three investigations (#2, #6, and Miami Beach) no new samples are listed

The department is currently conducting eight active investigations.

On Aug. 23, the department had enough information to close two of the ongoing investigations in Miami-Dade County, both were determined to be single cases with no additional transmission or linkage to areas of active transmission.

Aedes Aegypti Mosquito

Zika fever is a mild febrile illness caused by a mosquito-borne virus similar to those that cause dengue and West Nile virus infection. It has been identified in several countries in Central and South America, Mexico, and the Caribbean since 2015. Outbreaks have previously been reported in Africa, Southeast Asia, and the Pacific Islands. Local transmission has also been reported in Puerto Rico. Cases of Zika fever have been reported in travelers returning to the United States.

Information on Limited Local Transmissions

Florida has confirmed local transmissions of Zika virus are occurring in two small areas in Miami-Dade County. Area One is a small, less than one-square mile area in Miami-Dade County, just north of downtown. The department has found no evidence of transmission of Zika around nearly the entire perimeter of the area in Wynwood. This does not change CDC’s guidance for pregnant women or the Wynwood area. See Map 1 below.

Area Two is a less-than 1.5 square mile area in Miami Beach within the boundaries of 8th and 28th streets. Map 2 below indicates the impacted area in Miami Beach.

Florida’s small case cluster is not considered widespread transmission. If the department identifies additional areas of concern, we will notify the media and the public immediately.

The department is conducting 13 active investigations, including 10 in Miami-Dade, one in Pinellas and two in Palm Beach counties. The department continues door-to-door outreach and targeted testing in Pinellas, Palm Beach and Miami-Dade counties and mosquito abatement and reduction activities are also taking place around the locations that are being investigated. 

The department has closed out three investigations including two in Miami-Dade County and one in Broward County. These cases were determined to be single cases with no additional transmission or linkage to areas of active transmission.

On Sept. 7, the state health department, in response to questions from the Herald, said eight out-of-state residents have contracted Zika in Florida but are not included in the total count.

The agency also has stopped issuing details of active investigations into local Zika cases. On Sept. 1, the health department began reporting only a summary of the total number of investigations in each county. Previously, the agency had listed each investigation by county, with the number of people tested for Zika and the results of those tests.

Gambineri explained the change in an email: “The department is working to ensure the information is easy to understand. It had become confusing.”
