niman Posted January 5, 2016 Author Report Share Posted January 5, 2016 Human infection with avian influenza A(H5N6) virus – ChinaDisease outbreak news 4 January 2016Between 30 December 2015 and 2 January 2016, the National Health and Family Planning Commission (NHFPC) of China notified WHO of 2 additional laboratory-confirmed cases of human infection with avian influenza A(H5N6) virus.Details of the casesThe first case is a 26-year-old female from Baoan District, Shenzhen City, with onset date of 24 December. The patient was admitted to hospital on 27 December and is now in critical condition.The second case is a 40-year-old female from Duanzhou District, Zhaoqing City, with onset date of 22 December. The patient was admitted to hospital on 28 December and is now in critical condition.Public health responseThe Chinese Government has taken the following surveillance and control measures:making every effort to treat the patient; collecting and testing the specimens of the patient, carrying out viral isolation and whole genome sequencing and comparison;conducting epidemiological investigation; tracing, managing and observing the close contacts of the patient;strengthening surveillance of unexplained pneumonia and routine sentinel surveillance of influenza; strengthening the etiological surveillance of influenza/avian influenza virus.WHO risk assessmentWHO continues to closely monitor the influenza A(H5N6) situation and conduct risk assessments. So far, the overall risk associated with avian influenza A(H5N6) viruses has not changed.WHO adviceWHO advises that travellers to countries with known outbreaks of avian influenza should avoid poultry farms, or contact with animals in live bird markets, or entering areas where poultry may be slaughtered, or contact with any surfaces that appear to be contaminated with faeces from poultry or other animals. Travellers should also wash their hands often with soap and water. Travellers should follow good food safety and good food hygiene practices.WHO does not advise special screening at points of entry with regard to this event, nor does it currently recommend any travel or trade restrictions. The patient lives in Bao'an, Shenzhen. She is currently hospitalised for treatment and in a critical condition. A spokesman for the DH said, "All novel influenza A infections (including H5N6) are notifiable diseases in Hong Kong. "Locally, we will remain vigilant and work closely with the World Health Organization and relevant health authorities to monitor the latest developments." "In view of cases confirmed on the Mainland, members of the public should maintain good personal, food and environmental hygiene at all times during travel," the spokesman urged. "All boundary control points have implemented disease prevention and control measures. Thermal imaging systems are in place for body temperature checks of inbound travellers. Suspected cases will be immediately referred to public hospitals for follow-up investigation," the spokesman added. Regarding health education for travellers, the display of posters and broadcast of health messages in departure and arrival halls, environmental health inspection and provision of regular updates to the travel industry via meetings and correspondence are proceeding. Travellers, especially those returning from avian influenza-affected areas with fever or respiratory symptoms, should immediately wear masks, seek medical attention and reveal their travel history to doctors. Healthcare professionals should pay special attention to patients who might have had contact with poultry, birds or their droppings in affected areas. The public should remain vigilant and take heed of the advice against avian influenza below:* Do not visit live poultry markets and farms. The report of the first case of H5N6 flu cases in Shenzhen, the world's fifth H5N6 flu cases. After cases of Shenzhen Center for Disease Control and BGI cooperation, immediately outside the patient throat swab samples and live bird markets isolated from environmental samples of H5N6 influenza virus were sequenced. After fighting day and night, at 10:00 on January 3, it completed the full genome sequencing. Gene sequencing results showed that patients in the sample H5N6 virus HA, NA gene in 2014 in Guangdong Province, only isolated from duck H5N6 virus is highly homologous to the homology> 98%; and outside Shenzhen in 2014 live bird market environment isolated H5N6 virus is highly homologous to the homology> 98%; and Shenzhen in December 2015 3 H5N6 virus samples isolated from the external environment of highly homologous. Meanwhile, the patient sample H5N6 virus HA, NA gene in 2014, Sichuan and Guangdong reported two cases of H5N6 flu virus is highly homologous to the homology of> 98%. Internal patient sample H5N6 virus six genes and local external environment highly homologous H9N2 virus, H9N2 virus for the internal skeleton of the virus donor. Patient sample H5N6 virus HA and NA genes were derived from the extracellular environment of the H5N1 virus and the H6N6 virus. After sequencing analysis that the current market environment, the presence of outside Shenzhen live bird flu virus H5N6, reported cases of this virus and live bird markets outside environment is highly homologous virus was confirmed in this case it is due to contact with live poultry or from the perspective of bioinformatics external market environment infection. Therefore, reducing the chance of people in close contact with live birds, which can effectively prevent human infection with H5N6 influenza. One important measure is the effective implementation of central slaughtering of live poultry, cold chain distribution, fresh market. This measure is widely recognized in the international community, developed countries in the 1920s and 1930s began to promote fresh poultry market, at present, the developed countries and Hong Kong and Macao regions of Europe, America, Japan, etc., have implemented fresh poultry instead of live poultry market.
