Wuhan Municipal Health and Health Commission's Report on Unexplained Viral Pneumonia

    After the pathogen of "unknown cause of viral pneumonia" was initially determined to be a new type of coronavirus, the national, provincial and municipal expert groups immediately revised and improved plans for diagnosis, treatment, and monitoring of unexplained viral pneumonia. The Wuhan Municipal Health and Health Committee has organized tests on existing patient specimens. CHP provides latest information on cluster of pneumonia cases in Wuhan

     The Centre for Health Protection (CHP) of the Department of Health (DH) today (January 11) received notification from the National Health Commission (NHC) on the latest information of the cluster of pneumonia cases detected in Wuhan, Hubei Province. The public is again urged to maintain strict personal, food and environmental hygiene both locally and during travel.
 
    After the preliminary determination that the pathogen of "viral pneumonia with unknown cause" is a novel coronavirus, the National and Provincial Expert Groups have immediately revised and improved the protocols on diagnosis, treatment and surveillance of viral pneumonia with unknown cause. The Wuhan Municipal Health Commission has arranged the samples of existing patients to be tested for nucleic acid of the pathogen, and the Expert Groups have made overall assessments on the patients hospitalised for observation and treatment, taking into considering of information on clinical picture, epidemiological information and laboratory testing results, etc. As at yesterday (January 10), 41 patients have been diagnosed to have infection of the novel coronavirus. Among them, two patients have been discharged, seven patients are in serious condition and one died, while the remaining patients are in stable condition. A total of 739 close contacts have been identified and 419 of them are healthcare workers. All have been put under medical surveillance, and no related cases have been detected so far. There have been no new cases since January 3. For the time being, the Mainland's investigation has neither identified any infection of healthcare workers nor definite evidence of human-to-human transmission.
 
     According to information from the NHC, investigation, epidemiological investigations revealed that the patients are mainly business operators at a market called "Hua Nan Seafood Wholesale Market" in Wuhan, which has been closed since January 1.
 
     The Port Health Division (PHD) under the CHP has strengthened port health measures and enhanced temperature screening of travellers with increased frequency of random checking at all boundary control points. Additional thermal imaging system has been put in place at the Hong Kong International Airport dedicated for temperature screening of travellers from flights arriving Hong Kong from Wuhan.
 
     Moreover, additional manpower has been assigned to enhance temperature screening of inbound travelers when the daily two high speed trains with stop at Wuhan arrive at Hong Kong to ensure that body temperature of all passengers from these two trains are checked. Fever patients with acute respiratory symptoms who had visited Wuhan within 14 days prior to the onset of the illness will be immediately referred to public hospitals for isolation, treatment and follow-up.
 
     The PHD has advised the MTR Corporation Limited, the Airport Authority Hong Kong, and relevant airlines to enhance cleaning and disinfection measures of the high speed trains and planes from Wuhan as well as the cleaning of West Kowloon Station of the Guangzhou-Shenzhen-Hong Kong Express Rail Link and Airport Terminal Building. Moreover, dissemination of health messages through broadcast to travellers at e.g. flight cabin/West Kowloon Station, and distribution of leaflets, promulgating travel health news on website and posters display, has been enhanced. The PHD will continue to maintain close liaison with stakeholders of all boundary control points to timely update them about the latest situation and relevant measures.
 
     A dedicated webpage (www.chp.gov.hk/en/features/102465.html) has been set up to provide relevant information and health advice. To address the public's concern, the CHP will announce the details of cases fulfilling the reporting criteria (www.chp.gov.hk/files/pdf/enhanced_sur_pneumonia_wuhan_eng.pdf) on the above webpage daily to enhance transparency.
 
     To prevent pneumonia and respiratory tract infection, members of the public should always maintain good personal and environmental hygiene. They are advised to:

• Perform hand hygiene frequently, especially before touching the mouth, nose or eyes; after touching public installations such as handrails or door knobs; or when hands are contaminated by respiratory secretion after coughing or sneezing;
• Wash hands with liquid soap and water, and rub for at least 20 seconds. Then rinse with water and dry with a disposable paper towel or hand dryer. If hand washing facilities are not available, or when hands are not visibly soiled, performing hand hygiene with 70 to 80 per cent alcohol-based handrub is an effective alternative;
• Cover your mouth and nose with tissue paper when sneezing or coughing. Dispose of soiled tissues into a lidded rubbish bin, then wash hands thoroughly; and
• When having respiratory symptoms, wear a surgical mask, refrain from work or attending class at school, avoid going to crowded places and seek medical advice promptly.

     The public should take heed of the health advice below when travelling outside Hong Kong:

• avoid touching animals (including game), poultry/birds or their droppings;
• avoid visiting wet markets, live poultry markets or farms;
• avoid making close contact with patients, especially those with symptoms of acute respiratory infections;
• do not consume game meat and do not patronise food premises where game meat is served;
• adhere to food safety and hygiene rules such as avoiding consuming raw or undercooked animal products, including milk, eggs and meat, or foods which may be contaminated by animal secretions, excretions (such as urine) or contaminated products, unless they have been properly cooked, washed or peeled;
• if feeling unwell when outside Hong Kong, especially if having a fever or cough, wear a surgical mask, inform the hotel staff or tour escort and seek medical advice at once; and
• after returning to Hong Kong, consult a doctor promptly if having a fever or other symptoms, inform the doctor of recent travel history and wear a surgical mask to help prevent spread of the disease.

    A: The clinical manifestations are systemic symptoms such as fever and fatigue, accompanied by dry cough, and dyspnea in hospitalized patients is more common. Vital signs were generally stable at the time of admission.

 

    Q: There is currently one death case. What is the specific situation?

    A: The patient was a 61-year-old male. He was admitted to the hospital due to respiratory failure and severe pneumonia. He also had abdominal tumors and chronic liver disease. The patient purchases goods from Wuhan South China Seafood Market all year round. After admission to the hospital, symptomatic support, anti-infection, ventilator-assisted breathing, continuous ECMO in vitro life support, and other treatments did not improve his symptoms. A new virus implicated in a pneumonia outbreak in central China has killed a 61-year-old man there, Chinese health authorities said Saturday.

Chinese scientists investigating the outbreak discovered a new strain of coronavirus this week, and preliminary investigations found that 41 people in the Chinese city of Wuhan had been infected with it, the Wuhan health commission said.

There were no infections found among people in close contact with the infected patients, including medical workers, according to the commission. No new cases have been reported since Jan. 3.

Authorities have quarantined 59 people in the viral-pneumonia outbreak. Seven are in critical condition and eight have been released, the health commission said. Two of those eight are confirmed to have been infected with the new coronavirus, the commission said.

Man infected with mysterious virus in China dies after outbreak

 
 

SHANGHAI (Reuters) - A 61-year-old man has died from pneumonia in the central Chinese city of Wuhan in an outbreak of a yet to be identified virus while seven others are in critical condition, the Wuhan health authorities said on Saturday.

In total, 41 people have been diagnosed with the pathogen, which preliminary lab tests cited by Chinese state media earlier this week pointed to a new type of coronavirus, the Wuhan Municipal Health Commission said in a statement on its website.

Two of them have been discharged from hospital and the rest are in stable condition, while 739 people deemed to have been in close contact with the patients have been cleared, it said.

The man, the first victim of the outbreak that began in December, was a regular buyer at the seafood market, who had been previously diagnosed with abdominal tumors and chronic liver disease, the health authority said.

Treatments did not improve his symptoms after he was admitted to hospital and he died on the evening of Jan. 9 when his heart failed.

The commission added that no new cases had been detected since Jan. 3.

 

The Wuhan health authority also said that the patients were mainly vendors and purchasers at a seafood market in the city, and that to date no medical staff had been infected, nor had clear evidence of human-to-human transmission been found.

The World Health Organization said on Thursday that a newly emerging member of the family of viruses that caused the deadly Severe Acute Respiratory Syndrome (SARS) and Middle East Respiratory Syndrome (MERS) outbreaks, could be the cause of the present outbreak.

Coronaviruses are a large family of viruses that can cause infections ranging from the common cold to SARS. Some of the virus types cause less serious disease, while some like the one that causes MERS, are far more severe.

The outbreak comes ahead of the Lunar New Year holidays in late January, when many of China’s 1.4 billion people will be traveling to their home towns or abroad. The Chinese government expects passengers to make 440 million trips via rail and another 79 million trips via airplanes, officials told a news briefing on Thursday.

The Wuhan health authority in its statement also urged the public to take more precautions against infectious diseases, and said it was pushing ahead with tests to diagnose the pathogen and as of Friday had completed nucleic acid tests.

 

Hong Kong’s Department of Health said in a separate statement on Saturday that it strengthened checks and cleaning measures at all border check points, including the port, airport and the city’s high-speed rail station which receives passengers from Wuhan city.

In 2003, Chinese officials covered up a SARS outbreak for weeks before a growing death toll and rumors forced the government to reveal the epidemic. The disease spread rapidly to other cities and countries. More than 8,000 people were infected and 775 died.

BEIJING (AP) — Health authorities in the central Chinese city of Wuhan on Saturday reported the country's first death from a new type of coronavirus.

The Wuhan Municipal Health Commission said that seven other people were in critical condition.

It said a total of 41 were suffering from pneumonia caused by a "preliminarily determined new type of coronavirus” as of Friday, down from the earlier figure of 59. It said those were in stable condition and at least two had been released from a hospital.

The patient who died was identified as a 61-year-old man who had been admitted after suffering shortness of breath and severe pneumonia. The commission said he also suffered from abdominal tumors and chronic liver disease and had been a frequent customer at a food market on Wuhan's outskirts that has been linked to the majority of the cases.

It said he died after his heartbeat stopped on Thursday night.

China says the cause of the Wuhan outbreak remains unknown, but has sought to quash speculation that it could be a reappearance of the SARS epidemic, which killed hundreds in 2002 and 2003.

Coronaviruses are a large family of viruses, some of which cause the common cold, while others found in bats, camels and other animals have evolved into more severe illnesses.

Common symptoms include a runny nose, headache, cough and fever. Shortness of breath, chills and body aches are associated with more dangerous kinds of coronavirus, according to the U.S. Centers for Disease Control and Prevention.

Among the 41 cases that have been confirmed so far, 2 persons have been discharged from hospital, 7 are in critical condition, and 1 is reported dead.

The virus that caused the outbreak was identified as a new type of coronavirus; SARS, MERS, and bird flu have all been ruled out. No new case has been detected since [Fri 3 Jan 2020].

In the past 30 minutes there have been several reports of this death on Weibo, a Chinese social media platform. The above report as well as the Weibo reports mention there remain 7 critical patients out of 41 confirmed cases, (presumably eliminating the 19 cases reported to have been negative for the virus (see section [2] below), but adding one more case to the original stated 59 suspected cases in this cluster. It appears as though this fatality is not among the original reported 7 critical cases.

A new coronavirus identified by Chinese scientists is the putative cause of an outbreak of unusual pneumonia in the central city of Wuhan, according to Chinese news reports yesterday [Thu 9 Jan 2020]. In an interview today [Fri 10 Jan 2020] with Science, Xu Jianguo, head of an evaluation committee advising the Chinese government, confirmed that scientists have a complete sequence of the novel virus's genome.

The World Health Organization on [9 Jan 2020] requested sequence data, a spokesperson in Geneva says, and many scientists urge the country to make the sequence public quickly, but the decision is up to the top leadership of the Chinese Center for Disease Control and Prevention, says Xu, who is director of the Beijing-based State Key Laboratory for Infectious Disease Prevention and Control, part of China CDC. (The center's head, George Gao, did not respond to emails from Science seeking comment.)

Xu says the investigation is being led by China CDC but numerous groups in other government agencies are involved. "Plenty of people are working on the outbreak," he says. The role of the evaluation committee Xu leads is to review all the findings and make recommendations to the National Health Commission. Xu also said the novel coronavirus resembles known bat viruses, but not the coronaviruses that cause severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome (MERS).

The following transcript has been edited for clarity and brevity.
[Q=question, A=answer]

Q: The virus has been isolated from one patient, is that correct?
A: Correct. Two groups isolated the virus from samples from one patient. The viruses are nearly identical in morphology under electron microscopy. Researchers did laboratory investigations of 34 patients. A total of 15 were positive for the novel virus, [based on] sequencing samples of [fluid injected into the lung and collected for examination]. The teams got complete genome sequence data from about 10 patients. They are now attempting to isolate the virus from those samples as well. There are 19 cases with no evidence of the virus. There is no information available for the results of the remaining 25 cases.

Q: How close is this new virus to the SARS coronavirus?
A: The virus is similar to some of the published viruses collected from bats. But it is not close to SARS and not close to MERS.

Q: Are close contacts of patients and market workers being tested for antibodies to the new virus?
A: [Investigators] have just gotten the virus, they now need the chance to prepare reagents for antibody tests, but there are no data yet.

Q: The [Sun 5 Jan 2020] report from the Wuhan Municipal Health Commission, the latest available, says a total of 59 pneumonia patients have been identified as possibly carrying the virus. Have more patients been found?
A: It should be mentioned that the 59 reported pneumonia patients in Wuhan were clinically diagnosed; of those, 15 were confirmed to be infected by the new coronavirus. No new patients have appeared, as far as I understand. It's good news. People fear something like SARS in 2003, but this is a different case. The outbreak is limited, but we should test patients one by one [to identify] pneumonia caused by other pathogens.

Q: Are researchers trying to replicate the disease in lab animals to prove that it is really the cause of the outbreak?
A: People have recommended that [investigators] do tests to see if the virus can cause the infection in animals, but they need time.

Q: Is there any progress in tracing the original source of the virus?
A: I have no information. Personally, I'm interested, too. The virus looks like viruses isolated from bats, but how it was transmitted from bats to people is still a question. Several groups in China have been working on bat coronaviruses for years. I imagine they're working on this but so far there is no information.

Q: Are other live animal markets being checked?
A: The Wuhan market has been closed. I have no information about other [markets]. Wild animals carry the risk of exposing people to new viruses. I think we should have more strict regulations and inspections of markets that sell wild animals, especially since the source of the new coronavirus has not been identified and eliminated.

[A very interesting and helpful interview. In summary, 15 out of 34 individuals tested for this coronavirus had virus identified in specimens taken from them. A total of 19 of the 34 had no evidence of the virus on testing. No serology has been performed as yet as the serology testing hasn't been established as yet. It is curious that the virus is described as not being close to either the SARS-CoV or the MERS-CoV. Information from a reliable source had mentioned the virus had 20% diversion from the SARS-CoV. It will be interesting to see where the viruses are situated on a phylogenetic tree. By memory, when the SARS-CoV was 1st identified, the descriptions of the virus included a statement it was related to a bat coronavirus. And when the MERS-CoV was identified, a similar statement was made. Suggests concluding that when novel coronaviruses make a species jump, they possibly/probably started out as a bat coronavirus. So a question arises as to was this a direct bat to human jump, or is there an intermediate host facilitating this species jump?

We continue to see reports on suspected cased (presumably defined as a patient with a febrile respiratory illness with a history of travel from Wuhan City in the 14 days prior to onset). The majority of patients have had other known respiratory viruses isolated from specimens, and thus far, none of these suspected cases have given a history of visiting the Seafood market while in Wuhan City. That being said, in the absence of laboratory testing for this novel coronavirus, it is only speculation that they weren't infected with the Wuhan novel coronavirus.

Pressure builds on China to share info on new coronavirus

On Twitter and other forums, pressure is growing on China to share genetic sequences and other details about the new coronavirus implicated this week in an unexplained pneumonia outbreak in Wuhan.

So far, the only official announcements on cases came from Wuhan's health commission, which hasn't released any updates since [Sun 5 Jan 2020]. The announcements of preliminary identification of a new coronavirus have come mainly from tightly controlled Chinese media reports quoting Chinese scientists close to the investigation.

On [Thu 9 Jan 2020], the World Health Organization (WHO) confirmed the basic facts based on information from China and said over the coming weeks, more comprehensive information is needed to understand the dynamics of the outbreak and the patients' clinical symptoms.

Concerned global health officials

A handful of countries in Asia have identified sick travelers coming from Wuhan, though none have been linked to the city's pneumonia cluster, which authorities have traced to a now-shuttered seafood market that also sold live animals and meat from wildlife.

Though Chinese authorities have said human-to-human transmission isn't a feature of the new disease, global health officials are nervous, given Wuhan's population of 11 million people and the outbreak coming just ahead a heavy travel season connected to the upcoming Lunar New Year holiday [Sat 25 Jan 2020].

Also, the outbreak is occurring against the backdrop of the winter uptick in respiratory virus illnesses such as colds and flu, and health officials are eager to know how to test for the new virus.

Rumors are swirling in the scientific community that Chinese researchers are preparing medical journal articles for submission. Jeremy Farrar, MD, PhD, director of Wellcome Trust, said on Twitter today [Fri 10 Jan 2020] that if the rumors are true and critical health information isn't being shared immediately with the WHO, "something is very wrong." Another Twitter user pointed out that the virus has reportedly been fully sequenced and the team from China should have already shared it on GenBank.

Others commenting in the thread, however, pointed out the problem isn't unique to China, with similar calls for more timely data sharing made in the early days of other emerging outbreaks, such Middle East respiratory syndrome coronavirus (MERS-CoV) and Ebola virus in West Africa.

Meanwhile, a few new details about the patients' illnesses came today from Hu Ke, a member of the treatment team who was quoted in CN Healthcare, an online healthcare media portal. FluTrackers, an infectious disease news message board, translated and posted the report.

Hu said most patients have mild-to-moderate disease, some have been discharged, and most are in stable condition. The main symptoms are fever, fatigue, and a dry cough. Some experience breathing problems as the disease worsens.

Lab tests in early illness showed normal or decreasing white blood cell counts, decreased lymphocytes, with elevated liver and muscle enzymes in some patients.

Chest x-rays showed that all patients had pneumonia with multiple ground glassy exudates seen in the lungs.

European groups issue risk assessment, guidance

Elsewhere, the European Centre for Disease Prevention and Control (ECDC) yesterday [Thu 9 Jan 2020] posted its 1st risk assessment on the Wuhan novel coronavirus outbreak, based on limited information.

It notes that 3 European Union airports have direct flights to Wuhan, with indirect flights at other hubs.

"Considering there is no indication of human-to-human transmission and no cases detected outside of China, the likelihood of introduction to the EU is considered to be low, but cannot be excluded," it said, adding that more epidemiological and lab information is needed to do a comprehensive risk assessment.

Public Health England (PHE) today also weighed in today [Fri 10 Jan 2020], saying the risk to the United Kingdom population is very low and the threat to travelers to Wuhan is low. It urged people to take simple precautions such as observing good hand and personal hygiene.

The PHE also posted guidance on the initial investigation of possible Wuhan-related cases and infection prevention for the new coronavirus.

China Reports First Death From New Virus

The coronavirus, which surfaced in the city of Wuhan, has put the region on alert, but there is no evidence that it can spread among humans.

HONG KONG — Chinese state media on Saturday reported the first known death from a new virus that has infected dozens of people in China and set off worries across Asia.

The Xinhua news agency cited the health commission in the central Chinese city of Wuhan, where the illness first appeared, in reporting the death. The health commission said the patient, a 61-year-old man, died on Thursday night.

Forty-one people have been found to have the new virus, a coronavirus, and seven patients are still in severe condition, the health commission said, according to the Xinhua report. Two patients have been discharged, it said.

There is no evidence that the virus can be spread between humans, according to the commission. The initial cases were linked to workers at a market that sold live fish, birds and other animals. More than 700 people who had close contact with patients, including 419 medical workers, have been put under observation, the commission said, adding that no additional cases have been found.

Reports of the virus — a mysterious, pneumonialike illness — first appeared last month and have since aroused fears around the region, where memories of the SARS epidemic 17 years ago, which began in China and killed more than 800 people worldwide, still linger.

The man who died was a regular customer at the market in Wuhan and had previously been diagnosed with abdominal tumors and chronic liver disease, said the city’s health commission.

While no new cases have been detected since Jan. 3, questions remain. Experts have said the minimum incubation period for some viral infections is 15 days, suggesting that it may be another week before authorities can determine the full extent of the illness.

Reports of the man’s death came just before the Spring Festival, China’s biggest holiday, which begins Jan. 25, and a time when hundreds of millions people travel across the country. That mass migration raises the potential of spreading and worsening any outbreak. According to government estimates, Chinese passengers will take three billion trips in the next five weeks for family reunions and tourism.

In recent days, authorities have stationed personnel at airports and train stations across the region to scrutinize people who have visited Wuhan. The market linked to the virus is less than one mile away from a train station. Precautions include checking passengers for fever.

Authorities in the Chinese city of Hong Kong and elsewhere in Asia have also stepped up scrutiny.

The Hong Kong Hospital Authority said on Saturday that seven new patients between the ages of 3 and 50 who visited Wuhan in the last two weeks and presented with fever, respiratory or pneumonia symptoms had been admitted to local hospitals in the past 24 hours for monitoring, bringing the total number of patient cases in the city to 60 since Dec. 31.

Forty-six of those patients have already been discharged and the rest are in stable condition, the authority added.

Also on Saturday, South Korean officials said the illness of a Chinese woman they had placed under isolation earlier in the week was unrelated to the coronavirus, according to the country’s Yonhap news agency. The woman had visited Wuhan and suffered from coughing the throat irritation.

The Chinese government has faced pressure to release more information about the virus. In 2002, after SARS, a dangerous respiratory disease, first broke out, the Chinese government tried to cover up the extent of the epidemic, severely undermining public trust. Some experts have argued that China should disclose more details about the recent outbreak, such as the transmission route, the incubation period and the ages and genders of the patients.

Chinese health officials have cautioned that more research is necessary to fully understand the new virus. The World Health Organization has praised Chinese health officials’ response to the latest outbreak.

Coronaviruses are a large family of viruses that infect animals and people, and some cause only the symptoms associated with the common cold. Health officials in China have said that symptoms of the new illness include high fever, difficulty breathing and lung lesions.

WUHAN, Jan. 11 (Xinhua) -- One patient with viral pneumonia has been confirmed dead in Wuhan, capital of central China's Hubei Province, and seven others were in critical condition, local health authorities said Saturday.

Tentative diagnoses showed 41 cases suffer from pneumonia caused by a preliminarily determined new type of coronavirus by Friday, according to the Wuhan Municipal Health Commission.

The rest were in stable condition, among which two have been discharged from hospital.

A total of 739 close contacts, 419 of which are medical staff, have been placed under medical observation and no related cases were found, the commission said.

The patients' condition and epidemic situation is currently controllable, said Wang Guangfa, a member of the national medical expert team dealing with the situation.

The proportion of severe cases is similar to that among common pneumonia cases, Wang noted.

Wuhan on Jan. 5 reported 59 viral pneumonia cases with causes unknown previously.
