Email More A A SINGAPORE: A total of 115 Zika infections has been confirmed in Singapore, including the first pregnant woman in the country to be diagnosed with the virus, the Ministry of Health (MOH) and National Environment Agency (NEA) said in a joint briefing on Wednesday evening (Aug 31). The authorities also identified a new potential cluster at Bedok North Avenue 3, as well as confirmed 24 new cases, most of which are from the existing cluster. Of the 24 new cases, authorities said 22 cases are from the Sims Drive / Aljunied Crescent cluster, which now also extends to Kallang Way and Paya Lebar Way. Two of the new cases have no known links to the affected area. One patient lives at Joo Seng Road and the other lives at Punggol Way. On the potential new cluster at Bedok North Avenue 3, MOH and NEA said that three cases were reported yesterday. The patients had symptoms showing within four days and therefore did not infect one another. One case in Bedok North has a link to Kallang Way, but the other two have no known links to the Sims Drive / Aljunied Crescent cluster. Authorities said this suggests a possible new cluster. NEA said it would begin mosquito control operations at the potential new cluster at Bedok North Avenue 3. The authorities also identified nine of the cases as a result of MOH's look-back testing of previous cases. The pregnant Zika patient lives in the Aljunied Crescent / Sims Drive area and was diagnosed on Wednesday, authorities said, adding that she displayed mild symptoms of the virus and was taken to KK Women's and Children's Hospital. They added that she lives with a household member who was also diagnosed as positive for Zika. Her doctor is following up closely with her to monitor her health as well as the development of the baby, authorities said. She will be referred to a maternal-foetal medicine specialist for counselling and advice. Head of the Division of Maternal-Fetal Medicine in the Department of Obstetrics and Gynaecology Professor Arijit Biswas said the general guidelines are for pregnant patients with Zika to go for checks every four weeks. TESTING FOR ZIKA On Wednesday morning, Health Minister Gan Kim Yong visited the National Public Health Laboratory where blood and urine samples of those suspected of having Zika are tested. Posting about the visit on Facebook, MOH said the samples are sent to the lab from various public healthcare institutions, such as the Communicable Disease Centre and hospitals, where patients are referred to from general practitioner (GP) clinics and polyclinics. The tests take about three hours to be completed before results are sent back to the healthcare institutions. In all, it takes about half a day for patients to receive their results. Palm Beach County Health Dept. closes investigation into first local Zika case Continues working second case linked to Lake Worth Charlie Keegan 6:40 PM, Aug 29, 2016 BOCA RATON, Fla. - The Florida Department of Health in Palm Beach County announced it has closed the investigation into one of two cases of Zika believed to be acquired locally. Dr. Alina Alonso said investigators determined the case from August 8 was a single case, and mosquitos did not transmit the disease in Palm Beach County. Governor Rick Scott had previously said the patient in that case traveled to the Wynwood area of Miami, where mosquitos had been actively transmitting the disease. Dr. Alonso did not say where, specifically, the patient acquired Zika. Investigators continue to study a second case of Zika in Palm Beach County. Last week, NewsChannel 5 confirmed a woman in Lake Worth acquired the virus. Alonso said it could be weeks before investigators finish the investigation into that case. “What some people don’t understand is that the testing for this is a little complicated,” she said. “We take more than one test. It’s not a positive, negative kind of thing, you have to put the whole history together, you have to put all the tests together, and then come up with your conclusion.” At a roundtable discussion about Zika, Scott announced a new non-travel related Zika case from Miami Beach. That makes 43 locally acquired cases of Zika in Florida. Scott stressed keeping Zika from spreading involves everyone in the state. With rain in the forecast, standing water can collect in birdbaths, old tires, and other places. Those containers make perfect breeding grounds for the type of mosquito that transmits Zika. By dumping out standing water, you can eliminate breeding grounds for mosquitoes. “You can control your destiny,” the governor said. “These are your mosquitoes, you’re going to breed these mosquitoes, and your neighbors are going to breed them. So if you have no standing water in your area, then the odds are, you have a very low chance of getting Zika.” Last week, representatives from the department of health visited a neighborhood in Lake Worth near where the latest Zika patient lives. Alonso said teams stopped at more than 170 residences, and spoke face to face with people living at more than 80 of those properties. At each stop, representatives educated residents about Zika and provided free bug spray and larvicide. Gaby Mendez said she was happy to hear from the Health Department. She now knows to be vigilant in keeping water from pooling around her garden. “I have to put attention everyday; more when it’s raining. Putting the buckets so there’s no water on top,” Mendez said. Only one of Tuesday's three new cases was linked to Miami Beach, which is known to be an active area for local Zika transmission. The health department said it was investigating the other two cases to determine where they occurred, but cautioned, "One case does not mean ongoing active transmission is taking place." The four individuals in Miami-Dade were identified within the Wynwood area and experienced symptoms of Zika in mid-July. On August 30, the Florida MoH reported a record number of separate local cases in Miami-Dade county. In addition to the Miami Beach cluster, 2 new local cases were cited as new investigations. The press release stated that the new cases raised the number of active investigations to 12, with a link to the active investigation page, but the page was not update and only contained the data as of August 29, which had 10 active investigations. The August 29 page had internal inconsistencies as well as a confusing notation of pending samples. Several pending had been posted in the pending column for over a week, suggesting they were positive in Florida DoH Zika antibody tests and were awaiting confirmation by the CDC. This interpretation was supported by FLorida DoH comments in the Aug 23 press release when four samples that had been pending for the Wynwood cluster were upgraded to positive. The press release noted that these four samples had been from cases symptomatic in mid-July, but were classified as pending because the CDC had not yet ruled out cross reactivity due to a Dengue or Chikingunya, which was unlikely because these cases were linked to a confirmed Zika cluster. Several other active investigations had similar samples which had been in the pending column for at least a week, including 2 samples in investigation #2 (21 samples had been collected, and 19 were negative, but 2 remained pending). On August 24 the Florida DoH added an asterisk to the pending column indicating " *Awaiting confirmatory testing from CDC to rule out infection." suggesting that these two samples had also been antibody positive. However, on August 25 the two pending samples were updated and moved to the negatively column, so all 21 collected samples were negative, but the asterisk remained on the pending column, which showed 0 pending samples. On August 26 Recombinomics requested an explanation for the change in status for the two samples in investigation 2 as well as other samples that had been pending for over a week (5 samples in investigation 5 and 2 samples in investigation 6). An explanation was promised, but still has not been received. On August 29 the five samples were removed from the pending column. The negative column was increased from 1 to 5, but the data for the investigation was the internally inconsistent with 6 samples collected and 5 classified (as negative). The status of the sixth sample was no longer reflected in the table, Similar internal inconsistencies were seen in investigation #8, where 133 were listed as collected, but status was given for 128 (110 negative, 1 positive, 17 pending). Updates will include a Zika case count by county and information to keep Floridians informed and prepared. There are 16 new travel related cases today, five in Miami-Dade, three in Palm Beach, two in Broward, two in Orange, one in Polk and three involving pregnant women.Please visit our website to see the full list of travel-related cases. There are three new non-travel related cases today in Miami-Dade County, one is associated with the Miami-Beach investigation. The department is currently conducting an investigation into the other two cases to determine where exposure occurred. If you are pregnant and must travel or if you live or work in the impacted area, protect yourself from mosquito bites by wearing insect repellent, long clothing and limiting your time outdoors. According to CDC guidance, providers should consider testing all pregnant women with a history of travel to a Zika affected area for the virus. It is also recommended that all pregnant women who reside in or travel frequently to the area where active transmission is likely occurring be tested for Zika in the first and second trimester. Pregnant women in the identified area can contact their medical provider or their local county health department to be tested and receive a Zika prevention kit. CDC recommends that a pregnant woman with a history of Zika virus and her provider should consider additional ultrasounds. Additionally, the department is working closely with the Healthy Start Coalition of Miami-Dade County to identify pregnant women in the impacted areas to ensure they have access to resources and information to protect themselves.
