The man first developed a fever on Jan. 3 while in Wuhan, and on his day of return visited a local doctor in Japan, mentioning his visit to the Chinese city. He visited another doctor last Friday since his fever did not abate and was hospitalized the same day. After concluding from an X-ray of his lungs on Tuesday that his condition with a fever of unknown origin could possibly deteriorate, the hospital notified the health ministry later that day. This is the second confirmed case of 2019-nCoV that has been detected outside of China, following confirmation of a case in Thailand on 13 January. Considering global travel patterns, additional cases in other countries are likely. Coronaviruses are a large family of viruses that cause illness ranging from the common cold to more severe diseases such as Middle East Respiratory Syndrome and Severe Acute Respiratory Syndrome. 2019-nCoV is a new strain that has not been previously identified in humans. Not enough is known about 2019-nCoV to draw definitive conclusions about how it is transmitted, clinical features of disease, or the extent to which it has spread. The source also remains unknown. WHO encourages all countries to continue preparedness activities. On 10 January, WHO published information on how to monitor for cases, treat patients, prevent onward transmission in health care facilities, maintain necessary supplies, and communicate with the public about 2019-nCoV. The information includes advice on how to maintain hand and respiratory hygiene, and safe food and market practices. Japan's health ministry said the patient, in his thirties, did not go to a seafood market there thought to be the epicentre of the outbreak. But the Centre for Health Protection (CHP) in Hong Kong said according to information from their Japanese counterparts, he had “contact with a pneumonia patient while he was in Wuhan.” The patient was hospitalised on January 10, four days after his return to Japan, with a persistent fever. Tests on the patient, who was released from hospital on Wednesday, confirmed he was infected with the new virus. The CHP said in light of the new information, doctors will be asked notify health authorities of any patients who have fever and respiratory symptoms if they have been to any mainland hospital, or had close contact with anyone confirmed to be infected with the novel coronavirus Previously, they only had to refer sick people who had been to Wuhan. “Travellers who fulfilled the above reporting criteria will be immediately referred to public hospitals for isolation, treatment and follow-up”, a CHP spokesman said in a statement. He added that “the CHP will issue letters to doctors and private hospitals to inform them of the revised reporting criteria. Risk Assessment Outbreaks of novel virus infections among people are always of public health concern. The risk from these outbreaks depends on characteristics of the virus, including whether and how well it spreads between people, the severity of resulting illness, and the medical or other measures available to control the impact of the virus (for example, vaccine or treatment medications). Chinese health authoritiesexternal icon have reported that most patients in China visited a large seafood and animal market. They additionally report that several hundred health care workers caring for outbreak patients are being monitored and no spread of this virus from patients to health care workers has been detected. They report no sustained spread of this virus in the community, but there are indications that some limited person-to-person spread may have occurred. Cases outside China have all occurred in travelers from Wuhan. There is much more to learn about how the 2019-nCoV virus spreads, severity of associated illness, and other features of the virus. Investigations are ongoing in China, Thailand and Japan. Based on current information, however, the risk from 2019-nCoV to the American public is deemed to be low at this time. This is the first time a patient has been confirmed in Japan.  According to officials, the man is a 30-year-old Chinese who lives in the prefecture and has a fever on the 3rd and returns from Wuhan on the 6th.
