At least half of 40 brain scans from Brazil that Dr. William Dobyns has examined meet criteria for a rare condition known as fetal brain disruption sequence, a previously rare and highly destructive condition, according to the Seattle Children’s hospital medical geneticist and pediatric neurologist. “The brain in a fetus is growing and growing and growing,” he explained. “Then something really bad happens. The brain shrinks and the skull collapses.”It’s a condition much more severe than the microcephaly linked to Zika early on, a disorder in which babies are born with smaller-than-normal heads, Dobyns said.   5. Press ReleasesDepartment of Health Reports First Case of Zika in Rhode IslandThe Rhode Island Department of Health (RIDOH) today announced the first confirmed case of Zika virus in the state. The individual who tested positive, a male in his 60s, had recently traveled to Haiti, where there is active mosquito-borne transmission of Zika. "We have been closely monitoring the Zika situation internationally and have been coordinating with Rhode Island healthcare providers for months. We were fully prepared for this first case," said Director of Health Nicole Alexander-Scott, MD, MPH. "While the risk to the public is very low, we are coordinating with doctors, especially those who work with pregnant women, on how best to identify symptoms and educate patients about prevention." Measures that RIDOH has taken to prepare include: • Established a Zika Task Force that includes fetal medicine specialists from Women & Infants Hospital in February; • Issuing regular briefs to Rhode Island healthcare providers with updated guidance and information on symptoms and specimen collection; • Coordinating patient specimen collection and shipment to the Centers for Disease Control and Prevention (CDC); and • Coordinating with the Rhode Island Department of Environmental Management for increased mosquito surveillance and larvaciding. Zika is spread primarily through bites from infected mosquitoes. It can also be spread sexually. "We don't expect locally-acquired cases here because the species of mosquitos that are currently known to transmit Zika are not found in Rhode Island," said Dr. Alexander-Scott. "However, Rhode Islanders who are pregnant or are considering becoming pregnant should avoid travel to countries where there is active transmission of the virus." The virus causes symptoms including fever, rash, joint pain, headache, muscle pain, and conjunctivitis (pink eye). Approximately 80% of those who have the virus do not have any symptoms. Symptoms typically appear within three to 14 days of infection. Zika is also associated with pregnant women giving birth to babies with microcephaly and other birth defects. Microcephaly is condition when a baby's head is smaller than expected. Babies with microcephaly often have smaller brains that have not developed properly. Pregnant women or women who are considering pregnancy and have potential for exposure to Zika (from either a mosquito or a sexual partner) should seek counseling from their healthcare providers. In January, RIDOH first alerted healthcare providers to consider Zika virus in symptomatic patients who had recently traveled to areas with active mosquito-borne Zika virus transmission. Since February 1, 2016, RIDOH has been submitting specimens to CDC for testing. The Rhode Island State Health Laboratories are currently developing the capacity to test for Zika virus in accordance with CDC protocols. To prevent exposure to Zika and other mosquito-borne diseases, especially while traveling to areas with active mosquito-borne Zika transmission, people should: • Use and reapply Environmental Protection Agency (EPA)-approved bug spray containing at least 20% DEET. • Wear long-sleeved shirts and long pants. • Use permethrin-treated clothing and gear. • Stay in buildings that use air conditioning, or have window and door screens. • Sleep under a mosquito net. According to the CDC, Massachusetts has had 7 similar travel-associated cases, and Connecticut has had 1. More information about Zika virus disease is available at the Department of Health's website.   8. The state's second case of the Zika virus has been confirmed in a Newaygo County man who contracted the mosquito-borne disease while traveling. The patient, described as a "young man" whose exact age was not available Tuesday, was infected while traveling to Honduras at the end of January. Dr. Jennifer Morse, medical director for District Health Department #10, said the man suffered from symptoms within about a week of returning to Michigan. Test results by the Centers for Disease Control and Prevention came back about six weeks later. TUESDAY - Reversing course, the state Department of Health said Tuesday afternoon that it would make public any confirmed birth defects linked to exposure to the Zika virus. The department also told the Democrat and Chronicle that none of the eight New York women who contracted Zika while pregnant have given birth to a child with microcephaly, the most common birth defect connected to the virus. The announcement comes after the Democrat and Chronicle noted that state health officials were unwilling to discuss birth outcomes related to Zika. That blog, which was published online Monday and in print Tuesday, appears unchanged below. # Citing privacy concerns, state health officials will not say if the Zika virus has led to any birth defects in New York. To date, 69 New Yorkers have been diagnosed with Zika, the infectious virus that has swept through the Western Hemisphere over the last year. All but one of those state residents acquired the virus while traveling in parts of the world where the mosquito-borne virus is active — primarily, the Caribbean, Mexico, Central America and South America. This figure included eight women who were pregnant at the time they became infection. The sole New York who didn't pick up the virus overseas is a state resident who acquired Zika through sexual contact with an infected man, one of a handful of people worldwide known to have become infected in this way. ROCHESTER DEMOCRAT AND CHRONICLE Zika an unlikely threat in upstate New York State health officials have verified that unusual occurrence but have declined to discuss the possibility of another: whether any of the eight pregnant women with Zika bore a child with birth defects. One of the sad realities of Zika is that infection of pregnant women can promote birth defects. Most notably, women in Brazil have given birth to thousands of babies with microcephaly, or an abnormally small head. The condition, which absent the Zika virus is relatively rare, can be accompanied by vision or speech impairments and developmental problems. Microcephaly is by far the most common birth defect associated with the virus, though experts said recently that evidence is emerging of other connected birth defects. A small number of microcephaly cases have been reported in Colombia, Panama, Cape Verde, French Polynesia and Martinique, and health experts are waiting to see whether this spring and summer will bring an eruption of microcephaly as was seen in Brazil last year. New York health officials, though, are taking a different approach. Asked several times last week by the Democrat and Chronicle if any of the New York Zika cases had led to microcephaly or other birth defects, the health agency declined to answer. To do so, they said, could risk revealing the identity of the pregnant women. "In instances where the number of cases is small, patient confidentiality prohibits DOH from disclosing this information," the health department said in a written statement on Friday. "This involves a very small sample size of only eight pregnant women." They're not unique in this position. The Health Insurance Portability and Accountability Act (HIPAA) demands that doctors and health-industry insiders be careful not to identify patients or release medical information without permission, which has made health officials very reluctant to release data. Even in cases where names and other personally identifying information are withheld, they often cite HIPAA in instances where the number of cases is in single digits. If we want to know what sort of impact Zika has had in New York state — or what impact it will have going forward, when the number of local cases could increase considerably — we may have to look elsewhere for details.

Office of CommunicationsTuesday, April 19, 2016 OSDH Working to Identify Potential Travel-Related Zika Virus Cases in OklahomaFor Release: April 19, 2016 – Tony Sellars, Office of Communications, 405-271-5601 State Health Department Implementing Preparedness and Testing for Zika VirusAs public health and medical professionals are learning more about the effects of the Zika virus, the Oklahoma State Department of Health (OSDH) continues to broaden efforts to work with health care providers to identify potential travel-related cases, conduct testing in the state public health laboratory, and expand mosquito surveillance programs to determine presence and burden of the type of mosquito that is most likely to spread Zika virus. Presently, the Zika virus is not being spread by mosquitoes in the US, but is spreading aggressively in regions of South and Central America and the Caribbean. As more travelers acquire Zika virus abroad and summer approaches, there is a potential for localized transmission to occur in the southern US, especially in states where the Aedes aegypti mosquito is plentiful. Collections of this type of mosquito in Oklahoma are not commonly recorded. The most significant health risk of Zika virus infection is among pregnant women and their unborn babies. A study by the Centers for Disease Control and Prevention (CDC) recently established that infection with Zika virus during pregnancy can result in fetal loss or severe birth defects involving the brain. Measures implemented by OSDH to identify potential travel-related Zika virus cases among Oklahoma residents include a screening tool for maternity clinics and protocols for health care providers to assess information on patients to determine if Zika virus testing is indicated. OSDH Acute Disease Service epidemiologists are available 24/7 to consult with health care providers statewide to determine if their patients meet the criteria for testing and arrange for testing to be conducted. OSDH has received confirmation of four travel-related cases affecting Oklahoma residents through test results provided by the CDC Arbovirus Laboratory in Ft. Collins, CO.One of the Oklahoma cases is a pregnant woman. The OSDH Public Health Laboratory has completed proficiency testing for Zika virus and has begun testing of specimens from Oklahoma patients to alleviate the delay in receiving results from the CDC. Zika virus is transmitted to people primarily through the bite of an infected Aedes aegypti mosquito. It can also be spread through sexual contact with males who have the virus or from mother to unborn child. The disease can cause fever, rash, muscle and joint aches and red eyes. These symptoms typically last several days to a week, and hospitalizations are rare. Most people exposed to Zika virus won’t develop any symptoms at all. “We are strongly recommending that pregnant women and spouses or sexual partners of women who are pregnant or might be pregnant, avoid traveling to areas where Zika virus has been identified if at all possible,” said Oklahoma State Epidemiologist Dr. Kristy Bradley. If travel cannot be avoided, persons in these high risk groups should rigorously practice mosquito bite precautions and notify their health care provider if any symptoms of illness occur within 2 weeks of travel. OSDH has also enlisted the help of the Oklahoma State University Department of Entomology to conduct surveillance in five urban regions across the state for the presence of Aedes aegypti mosquitoes. The Oklahoma City-County and Tulsa Health Departments also have plans to expand their mosquito surveillance to monitor for the presence of this Zika virus vector in addition to trapping and testing of Culex mosquitoes, which spread West Nile virus. The CDC and OSDH advise pregnant women to delay travel to foreign countries and U.S. territories where Zika virus is being transmitted. To prevent the spread of the disease, people traveling to those areas should carefully follow steps to avoid mosquito bites while there and for seven days after returning home. Mosquito exposure prevention tips while traveling to affected areas include: Use air conditioning or window/door screens to keep mosquitoes outside. Of those, 30 were in travelers who were infected abroad and diagnosed after they returned home; one of those travelers was a pregnant woman. One case involved a Dallas County resident who had sexual contact with someone who acquired the Zika infection while traveling abroad.
