Photo: AFP About 5 million residents left Wuhan before the lockdown because of the deadly coronavirus epidemic and the Spring Festival holiday, mayor Zhou Xianwang revealed on Sunday, as health officials warned the virus’ ability to spread was getting stronger. There were about 9 million people remaining in the city after the lockdown, Zhou told a press conference. Of the 2,700 people currently under observation in the city, about 1,000 were likely to be confirmed cases. As of Sunday, Wuhan had 533 confirmed cases. The central government imposed a lockdown on Wuhan and several cities on Thursday hoping to stop the new virus from spreading to other parts of the country. However, many had already left the city for the holiday, while others rushed out after the lockdown was announced on Wednesday night. China, meanwhile, said Premier Li Keqiang would head the high-level group to fight the coronavirus epidemic that has killed 56 people and infected more than 2,000 others, while health officials said the virus’s ability to spread is getting stronger. Beijing city reported five more confirmed cases, including a nine-month-old infant. It is the first confirmed infant case. On Saturday, a two-year-old girl in Guangxi was confirmed to be infected.

Beijing Municipal Health Commission, in an announcement on Jan. 26, stated that three of its doctors had tested positive for 2019-nCoV. One of the three doctors, surnamed Wang, traveled to Wuhan on a business trip on Jan. 8 and reported to Beijing on Jan. 16. He tested positive for the virus on Jan. 21. The second doctor, Lu, went to Wuhan on Jan. 10 and returned to Beijing the following day. He also tested positive on Jan. 21. The third doctor, surnamed Yan, recently went on trips to several areas in China, including the Chinese cities of Changchun and Dalian. During a meeting on Jan. 14, Yan sat next to Lu and began having a fever on Jan. 18. Yan tested positive for the virus on Jan. 25. The announcement said the three doctors are now under isolation treatment.

The patient, a “traveler from Wuhan, China, has been in contact with the HCA and was provided guidance in order to reduce exposure to the public while awaiting laboratory confirmation from the CDC. The individual has now been transported to a local hospital and is in isolation in good condition,” the HCA said in a statement. 2019 Novel Coronavirus: Update for Orange County January 23, 2020 An outbreak of respiratory disease caused by 2019 Novel Coronavirus (2019-nCoV) has been identified in Wuhan, Hubei Province, China. Over 500 cases and 17 deaths have been identified since the outbreak was identified in December. Traveler cases from Wuhan have been identified in other areas of China, including Beijing and Shenzhen, as well as Taiwan, Thailand, Japan, and South Korea. The first United States case was confirmed earlier this week in a Washington state traveler from Wuhan. The initial illnesses are believed to be related to animal exposure at the Wuhan South China Seafood City (also called the South China Seafood Wholesale Market and the Hua Nan Seafood Market), which was closed on January 1. More recent cases have demonstrated that person-to-person transmission occurs, and several cases have been seen in healthcare workers who cared for cases, though it’s unclear how easily or sustainably the virus is spreading between people. Because the largest number of cases have been identified in Wuhan, public health surveillance is currently focused on travelers from this area. Earlier this week, China announced that they would be restricting travel out of Wuhan, though it is unclear at this point how comprehensive these restrictions will be. CDC has implemented symptom screening of travelers arriving from Wuhan, at Los Angeles International Airport, San Francisco International Airport, and John F. Kennedy International Airport in New York. Screening will be implemented soon at Atlanta Hartsfield-Jackson International Airport and Chicago O’Hare International Airport. Most travelers from Wuhan to the United States will pass through one of these airports and receive a health assessment upon arrival. If this assessment reveals no evidence of acute illness, they will be educated about the current outbreak and instructed to seek medical care if they develop illness. Orange County providers may care for patients who develop illness following travel from Wuhan. Recommendations for Providers: Providers should immediately report all cases of suspected 2019-nCoV infection to the Orange County Health Care Agency at 714-834-8180 (after hours contact the OCHCA physician on call at 714-628-7008). Providers should take a detailed travel history for patients with fever and acute respiratory illness to identify potential exposures. If a patient reports recent travel to China, the provider should clarify which cities and provinces were visited. As this situation continues to evolve rapidly, providers should consistently monitor CDC recommendations at https://www.cdc.gov/coronavirus/2019-ncov/index.html. Clinical Presentation: Based on experience with other coronaviruses, 2019-nCoV’s incubation period is presumed to extend up to 14 days. The virus’s transmission dynamics have yet to be determined. The primary clinical symptoms have been respiratory, including fever, cough, and shortness of breath. A significant proportion of cases have had pneumonia. Cases of more mild illness are being identified as well; at this point it is not clear how likely severe illness is for persons infected. When to Test: Patients who meet the following criteria should be tested for 2019-nCoV: ----- 1) Fever and symptoms of lower respiratory illness (e.g., cough, shortness of breath) --------- AND in the 14 days before symptom onset: -------- History of travel from Wuhan City, China --------- -or- --------- Close contact with a person who is under investigation for 2019-nCoV while that ------------- person was ill. ----- 2) Fever or symptoms of lower respiratory illness (e.g., cough, shortness of breath) --------- AND in the last 14 days before symptom onset, close contact with an ill ------------------------- laboratory-confirmed 2019-nCoV patient Note that these recommendations should serve as guidelines for providers. But fever may not always be present, and clinical judgment should always be used to guide testing. OC Health Care Agency Confirms First Case of Novel Coronavirus in Orange County, California (Santa Ana, CA) – The OC Health Care Agency’s (HCA) Communicable Disease Control Division received confirmation from the Centers for Disease Control and Prevention (CDC) this evening that an Orange County, California case has tested positive for the novel coronavirus. The case, a traveler from Wuhan, China, has been in contact with the HCA and was provided guidance in order to reduce exposure to the public while awaiting laboratory confirmation from the CDC. The individual has now been transported to a local hospital and is in isolation in good condition.In consultation with the CDC and the California Department of Public Health, the HCA is following up directly with all individuals who have had close contact with the case and are at risk of infection. The HCA will be monitoring any close contacts and assure that proper evaluation and care is provided if they become ill. The CDC’s guidance indicates that people who have casual contact with a case (in the same grocery store or movie theater) are at minimal risk of developing infection. Ontario Confirms First Case of Wuhan Novel Coronavirus Extensive Protocols in Place to Detect and Contain Cases January 25, 2020 5:55 P.M. Ministry of Health TORONTO — Today, Dr. David Williams, Ontario's Chief Medical Officer of Health, announced Ontario's first presumptive confirmed case of Wuhan novel coronavirus in Toronto. On Thursday, January 23, 2020, Sunnybrook Health Sciences Centre admitted a patient brought in by paramedics who presented with fever and respiratory symptoms. The patient was screened, recent travel history to Wuhan, China, was confirmed and the patient was immediately put under isolation. The hospital and paramedic service took all necessary precautions to ensure the safety of staff and other patients. Diagnostic testing was conducted, and specimens were sent to the Public Health Ontario Laboratory to confirm the diagnosis. On Saturday, January 25, 2020, the Public Health Lab confirmed the case as a presumptive positive case. "All appropriate infection prevention and control measures were followed by both paramedics and the hospital," said Dr. Williams. "Toronto Public Health is conducting case and contact management and Ontario is in touch with our federal counterparts to help determine exposure to other individuals on the flights." Recently, to strengthen the province's ability to monitor any coronavirus cases, the Ministry of Health added novel coronavirus as a designated disease reportable under Ontario's public health legislation. Because of this, and other protocol and procedures that Ontario has in effect, the case was quickly identified and protocols for protection of health care workers and the public was immediately enacted. "Although there is now a presumptive confirmed case of novel coronavirus in Ontario, I want Ontarians to know that the province is prepared to actively identify, prevent and control the spread of this serious infectious disease in Ontario." said Elliott. "I want Ontarians to know that their health and wellbeing is my top concern. Ontario has robust processes in place, skilled clinicians and dedicated health workers to identify and manage this and any future cases safely and effectively. We have specialized units within our hospitals to deal specifically with these types of cases when they arise." Most of the cases are in and around Wuhan, in central China, but at least 19 others have been found as far away as Australia, France and the US. Three people are under observation in New York State, Gov. Cuomo said Friday.
